New Patient Information
We welcome all new patients and thank you for trusting us with your dental health. Please ask your former dentist to send a copy of your records to our office.
How to prepare for your appointment:
Please be sure to bring your dental insurance card and arrive early to fill out your initial paperwork if you have not already done so. (These forms can be downloaded above)
If you are on any medications, please bring a list of them with you to the office
If you need antibiotics prior to dental work, please take them as prescribed. If you are not sure if you need antibiotics, please contact your doctor prior to your visit

Digital x-rays have many advantages compared to film x-rays. First, they reduce the amount of radiation needed. The improved diagnostic capability of digital x-rays and the ability to view the x-rays on a computer screen allow the patient to better understand and follow treatment. Digital x-rays are both eco-friendly and a time saver! They are instant, with no need to develop film, saving the traditional supplies used and chemical waste.

Using Electronic Medical Records is a wonderful addition to our practice. We are able to quickly and accurately access patient information, ensuring patient confidentiality without excessive paper waste. This digital format allows for quick access to your information for both in-office and insurance purposes.

This advanced technology enables both you and your dentist to view detailed images of your teeth and mouth during your visit. Using an intraoral camera, you can gain a clearer understanding of what’s happening in your mouth and review the current state of your oral health.

Monitors are located next to each patient chair, allowing patients to view their dental x-rays while discussing the findings with the doctor, helping them gain a clearer understanding of their oral health.

We are happy to help you understand and maximize your out-of-network insurance benefits and will submit claims on your behalf whenever possible. For larger or more complex treatment plans, we also offer flexible payment options to help make your care more manageable.
