New Patient Information

We welcome all new patients and thank you for trusting us with your dental health. Please ask your former dentist to send a copy of your records to our office.


How to prepare for your appointment:

  • Please be sure to bring your dental insurance card and arrive early to fill out your initial paperwork if you have not already done so. (These forms can be downloaded above)

  • If you are on any medications, please bring a list of them with you to the office

  • If you need antibiotics prior to dental work, please take them as prescribed. If you are not sure if you need antibiotics, please contact your doctor prior to your visit

Being out-of-network also gives our doctors greater flexibility in treatment planning, materials, techniques, and technology, allowing us to maintain the high standard of care we believe our patients deserve.

We are happy to help you understand and maximize your out-of-network insurance benefits and will submit claims on your behalf whenever possible. For larger or more complex treatment plans, we also offer flexible payment options to help make your care more manageable.

Our team believes in complete transparency. Before beginning treatment, we will review your recommended care, estimated costs, insurance benefits, and available payment options so you can make informed decisions with confidence.
